Our Case Managers provide an array of services to assist individuals and families cope with complicated health or medical situations.

 

We help people to identify their goals, needs, and resources. Case manager helps clients to find resources and facilitates connection with services. We can advocate on behalf of a client to obtain needed services.

 

The case manager also maintains communication with the client to evaluate whether the plan is effective in meeting the client’s goals. We work together with individuals to achieve a better quality of life.

 

 

  
  Rehabilitation Services

Cambridge Health keeps our clients active, minimizes deconditioning, optimizes physical functioning, and enhances overall well-being.

 

Cambridge Health, Inc. consults with physiotherapists and occupational therapists for each of our client's physical rehabilitation plan. All of our client receive a individualized and therapist directed excercise progam to our clients.

At Cambridge Health, Inc. we tailor our services based on each client's individualism. We focus on the person's positive traits and recognizes the challenging behaviours are simply a way of communication. Our staff are trained and specializes in supporting our clients through those difficult times. We specializes managing behaviours such as:

 

  • Physical and/or verbal aggression

  • Behaviours associated acquired brain injury

  • Neurological disorders.

  • Combination of physical and psychological disorders

  

At Cambridge Health, Inc. we take pride operating our care facilities, all of our homes are clean, modern, and well maintained. Our staff are courteous, professional, and friendly. They ensure our care homes have a home-like environment where residents can receive tailored individualize services and have more personal attention.

 

Our Adult Residential Care Program provides individuals with comfortable, secure, private rooms.  The program includes 24 hours staff support, provide balanced meals, engaging social activities, and personalized mental / physical care plans.
